The merger of the Blakeney Surgery and Coleford Medical Practice websites to become Dean Medical Practice is currently taking place. While this work is carried out all forms on this website will be unavailable. We are very sorry for any inconvenience caused during this time. Forms will be made available as soon as the merge is complete.
The practice is occasionally supported by donations, which are retained and recorded separately.
These donations are used to purchase equipment to provide better facilities for patients. Any donations are therefore very welcome and will benefit everyone.
